code size optimizations
...
- make Profiler::readCounters() not inline as it didn't need to be,
it's not performance critical and it's sufficiently large.
- don't inline hasExtensions(), same reason.

Tweak Profiler API so it works in multithread
...
Instead of using a singleton, Profiler has to be instantiated in the
scope/thread it is used.
